To set up GTA 5 map patches, you first need to install the Map Editor program on your computer.
After downloading the Map Editor file, create a folder called "scripts" inside the game's installation directory and place the file from inside the downloaded archive there. Place the Heist file in the game's installation directory.
Once you are in the game, press the F7 key to open the map editor and go to the "Load maps" section. Select "Heist" from the file chooser and wait for loading to complete before starting to play.
