Open OpenIV.
Navigate to the following path in your game files: mods/update/x64/dlcpacks/custommaps/dlc.rpf/x64/levels/gta5/citye/maps/custommaps.rpf/
Drag and drop the downloaded YMAP file into this directory.

Launch the game to enjoy!
